New Patient Registration

Our practice offers a welcoming space for everyone who seeks to use our services.

All patients residing within our catchment area are welcome to register with us, you do not need proof of address or immigration status, ID, or an NHS number to do so.

Where possible we ask you to complete our online registration form (please see below), however, if you are unable to, please visit the practice reception to pick up our New Patient Registration Forms.

Please Note: If additional information is required for us to process your application we will contact you via your preferred method of contact, failure to respond may affect the timeliness of you getting you registered.

When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

From April 2016 a requirement of the new GP contract is that all patients, including children, must be allocated a named, accountable GP who takes responsibility to oversee the delivery of care.

Dr. Shah is the named GP for all patients registered at Thorpe Road Surgery.

Please note that this does NOT prevent you from seeing any other GP in the practice as you currently do.  Patients can and should feel free to choose to see any of the GPs in the practice in line with current arrangements
